Evaluation forms are a great way to assess candidates consistently. When evaluating, the assessor can assign ratings from 1 to 5 and add notes for each competency, along with an overall summary, ensuring a comprehensive evaluation process.
Understanding evaluation forms
The structure of a hiring evaluation form may vary depending on the organization and the specific requirements of the job position. However, a typical hiring evaluation form generally includes assesses topics like:
- Job Requirements: this section outlines the key skills, qualifications, and competencies required for the job. It serves as a reference point to evaluate the candidate's suitability for the position.
- Interview Evaluation: this section focuses on assessing the candidate's performance during the interview process. It may include subsections such as: Communication, technical, leadership or analytical among others.
How to create evaluation form templates
Before starting, make sure to have the "Manage evaluation form templates" permission enabled.
- On your sidebar, go to Settings
- Go to the People section and click on Recruitment
- Click on the Templates tab
- Scroll down to Evaluation form templates
- Click on New template or (…) to edit, duplicate and delete existing templates
- Add as many Requirement/skills as you need
- Save
Each evaluation is structured in the following way:
- Evaluation form title (eg. Cultural fit)
- Requirement or skill (eg. Teamwork)
- Requirement or skill description or interview questions

You can save as many templates as you need. You can customize templates for different departments, roles or levels.
How to set up evaluation forms in a job opening
- On your sidebar, go to Recruitment → Jobs
- Select a Job opening
- Click the Edit button
- Select Internal evaluation forms

- Create a new evaluation form from scratch
- Use an evaluation form previously created
- Click on Save

Once an evaluation form has been added to a job opening, it will be visible in the evaluation section of the applicant’s profile, it’s possible to assign ratings from 1 to 5 and add notes for each competency, along with an overall summary.
How to assess candidates using evaluation forms
- Select a job opening in order to view the applicants
- Select the applicant to evaluate by clicking on the arrow on the right side, this reveals the applicant’s profile
- Click on the Evaluations tab
- Fill in assigned form
- Save

When using the evaluation form:
- The hiring manager can preview the scorecard details, as it can be used as an interview kit.
- Hiring managers can add a rating and a note to each requirement or skill.
- Finally, before submitting, the hiring manager must add a summary.
- Once published, it will be visible to all hiring managers and can be viewed from the rating column in the applicants' list. Hover over the rating to see which evaluation form was completed and by whom.
It's possible to filter candidates by their rating, choosing from three different ratings, and the candidates with at least that rating will be displayed in the table.
